The state must prove that the defendant:

Willfully refused or failed to stop.
The officer had police insignia on his vehicle.
Defendant was operating a vehicle.
A duly authorized law enforcement officer ordered the defendant to stop or remain stopped.
Defendant knew he had been directed to stop.
